Referral Source Communication during Patient's Rehab Experience
The patient's initial drug rehab therapist will be your primary contact, although that may change as the patient nears discharge. You will always be in communication with the patient's drug rehab therapist, as well as, the patient. We realize that every referral source may have different needs, so it is critical you make us aware of the level of communication you require. Some sources of referral care to have communication on a weekly basis, others are comfortable with waiting until discharge is approaching.
Regardless of your communication needs, you may rest assured you will be included in any discharge planning, just as you will be notified if for some reason your patient terminates drug rehab prematurely. We want you to be part of our team.
