  • Do you suffer from any other addictions or medical conditions?
  • Do you suffer from a mental health disorder?
  • What is your medical history?

Answering these questions and others honestly will allow the physician to create a course of treatment that will provide you with the absolute best chance at achieving long-term recovery.
